1. Register with Leafwell, speak to an Iowa-licensed healthcare provider online, and get approved. Consultations cost $99 and you’ll only be charged if approved.
2. Download your doctor’s recommendation and submit it with your state application to the Iowa Medical Cannabis Program.
3. You will receive an email from the state when your application has been processed. Online applications are typically processed in one to two days, while applications received by mail can take one to two weeks.

Caregiver Requirements:

• Be at least 18 years of age.
• Be a resident of Iowa or a bordering state (Illinois, Minnesota, Missouri, Nebraska, South Dakota, or Wisconsin).
• If caring for more than one patient, you must submit a separate application for each patient.

To apply as a caregiver, follow this process:

1. You will need your associated patient’s completed Healthcare Practitioner Certification form with you listed as a caregiver on page 1.
2. Click on the Online Caregiver Registration button found on this page.
3. Select if you are a new or renewing caregiver.
   1. If renewing, you’ll be asked for your registration number and email address. The registration number can be found on your previously issued card.
4. Select if you are applying as a caregiver for a minor or adult patient.
   1. If for an adult patient, your associated patient must have a currently issued card for you to be approved. You’ll be asked to provide their registration number on the application.
   2. If for a minor patient, they do not need to apply as a patient. You will be issued a card on their behalf when approved.
5. Fill out the online application.
   1. If applying to be a caregiver for an adult patient, you’ll enter their registration number on the application.
   2. If applying to be a caregiver for a minor patient, you’ll have a section to complete their demographic information.
6. Upload required images.
   1. The patient’s completed healthcare practitioner form with you listed as a primary caregiver on page 1.
   2. A photo of the front of your valid driver’s license or state-issued non-operator’s ID.
7. Electronically sign the attestation and submit.
   1. Once you click submit, an attestation statement will appear. Enter the same email you used on the application, and check the box to complete the electronic signature.
8. Complete your payment.
   1. After signing the attestation and submitting you’ll be directed to a payment screen to pay the $25 caregiver registration fee.
9. When your application is reviewed you will receive an email stating that you are either approved or pending information.
   1. If Approved, you’ll receive an approval email with your valid digital card attached as a PDF. Your permanent card will be mailed to the address on your application.
   2. If Pending Information, you’ll receive an email stating which items are missing, and you can upload the missing items or make a payment using a link in that email.

After you’ve been approved, you will receive your Healthcare Practitioner Certification Form via email, and then you can apply to the state. This process is the same for new and renewal patients.

Before you start, have the following ready:

• Photo ID (Iowa Driver’s License or Non-operator State ID)
• Healthcare Practitioner Certification Form (emailed to you from Leafwell)
• Debit card, credit card, or e-check
  • Proof of eligibility for reduced fee, if applicable

Getting your Iowa medical marijuana card:

The state recommends using Chrome, Safari, and Edge as your browser.

1. Go to the Iowa Medical Cannabis website and click “Online Patient Registration.”
2. Select if you are a new or renewing patient.
   1. If you are renewing, you will be asked to provide your registration number and email address. Your registration number can be found on your previous card. You will want to use the same email you used the previous year.
3. Fill out the online application. Double-check for errors with your name, date of birth, and driver’s license number before submission.
4. Upload the required documents. The document upload buttons are further down on the adult patient application. Click the “choose file” buttons to select and upload your file.
5. Once you click submit, an attestation statement will appear. Enter the same email you used on the application, and check the box to complete the electronic signature and submit.
6. Complete your payment.
7. When your application is reviewed you will receive an email stating whether your application is approved or pending information.
   1. If approved, you will receive an approval email with your valid digital card attached as a PDF.
   2. If pending information, you will receive an email stating which items are missing. Use the link in that email to either upload missing items or complete payment.
