    Adults at least 18 diagnosed with at least one qualifying medical condition may be eligible to apply for a Florida medical marijuana card.

    Existing Leafwell patients can renew their medical marijuana cards online. Patients new to Leafwell must see a healthcare provider in person.  Stay tuned for upcoming events!

    Caregiver

    Caregivers must be at least 21 years of age and a Florida resident. They must agree in writing to assist with the qualified patient’s medical use of marijuana and complete a caregiver certification course provided by the Florida Department of Health or its designee.

    Can Minors Qualify?

    Yes, but if the patient is younger than 18 years of age, a second qualified physician must concur with the determination that the medical use of marijuana would likely outweigh the potential health risks. The second physician certification must be documented in the patient’s medical records.

    Only a caregiver may purchase or administer medical marijuana for a qualified patient younger than 18.

What You Need

You’ll need to provide the following to apply for the FL medical marijuana program:

    Medical Marijuana Recommendation

    Once approved, you will receive two emails from the state. One includes your username (email), and the other includes a temporary password that expires in 24 hours. Use those credentials to log into the Florida Medical Marijuana Program website to complete your online application.

    Proof of Identity

    You will need a government-issued photo ID, such as a Florida driver’s license or Florida state identification card. You may also submit an out-of-state driver’s license or state identification card with two proofs of Florida residency (if you’re a seasonal Florida resident). For minors,  a certified birth certificate or FL school registration record, AND legal guardian’s FL Driver’s License or State ID.

    Proof of Florida Residency (Optional)

    Permanent adult Florida residents must only submit a copy of a valid Florida driver’s license or Florida state ID card.

    A seasonal Florida resident without a valid Florida driver’s license or Florida ID card must submit a copy of two of the following documents:

    • A deed, mortgage, monthly mortgage statement, mortgage payment booklet, or residential rental or lease agreement.
    • One proof of residential address from the seasonal resident’s parent, step-parent, legal guardian, or another person with whom the seasonal resident resides and a statement from the person with whom the seasonal resident lives stating that the seasonal resident does live with them.
    • A utility hookup or work order dated within 60 days of registration in the medical use registry.
    • A utility bill up to two months old.
    • Mail from a financial institution, including checking, savings, or investment account statements up to two months old.
    • Mail from a federal, state, county, or municipal government agency up to two months old.

Next Steps After Approval

Congratulations on getting your medical marijuana certification! You’re almost there, but there are a few more steps to complete to get your Florida medical cannabis card. Let’s dive in!

  • Before You Start: What You’ll Need
    • A credit or debit card for payment
    • A valid photo ID
  • Receive Emails: Retrieve your login credentials for the Medical Marijuana Use Registry from your email. If you don’t find the emails from the OMMU in your inbox, check your spam and junk folders.
  • Complete Application: Access the “Your Card” page in the Medical Marijuana Use Registry to complete your online application.
  • Pay the Fee: Supply all necessary documentation and submit your payment information for the $75 processing fee (a $2.75 convenience fee will also be added for online payments).

Receive Your Card: In 5-10 business days, you will receive email approval from the Florida state agency, including a temporary card to purchase cannabis from a licensed medical marijuana dispensary. Physical medical marijuana cards will arrive two weeks later via mail.

How to Renew Your Card

It’s easy to renew your Florida medical marijuana card with Leafwell.

You must renew 45 days before your card expires, and Florida patients can only be seen by their previously approved provider via telemedicine.

Navigating the legal landscape can be tricky, but we’re here to break it down for you. Here are some key points you should know if you have a medical marijuana ID card in Florida.

No Public Consumption: Patients can not consume cannabis in public places or on public transportation.

Cultivation is Off-Limits: Patients cannot cultivate cannabis plants.

Medical Marijuana Treatment Center: Licensed medical marijuana treatment centers (MMTCs) are vertically integrated and are the only businesses in Florida authorized to dispense medical marijuana and low-THC cannabis to qualified patients and caregivers. They are also commonly referred to as medical marijuana dispensaries or clinics.

Daily Dose Amount Limit: According to a 2022 emergency rule “a qualified physician may not issue a physician certification for more than three 70-day supply limits of marijuana or more than six 35-day supply limits of marijuana in a form for smoking.”

Recreational Use Is Prohibited: Florida Medical Marijuana Laws prohibit anyone in Florida to possess marijuana flower or low-THC cannabis unless it is sealed in a tamper-proof container sold at a medical marijuana clinic.

Caregivers

If you’re looking to become a medical marijuana caregiver for a medical marijuana patient in Florida, there are some important rules and steps to follow.

Picking Up for Patients

As a caregiver, you can pick up medicine from a medical marijuana dispensary on behalf of a patient with terminal illnesses or in hospice care.

Caregiver Limitations

If a caregiver is not a close relative of the qualified patient they are assisting, they must also pass a background screening. If the patient is a minor, having a caregiver is a must.

Application Process

When applying to become a caregiver, applicants must provide the following:

  • A completed caregiver application.
  • A copy of the caregiver’s proof of residency.
  • A $75 check or money order to cover the application fee payable to the Florida Department of Health.
    • A $2.75 convenience fee applies to online payments.
  • A 2×2-inch photograph taken within 90 days. The photo must be passport style, in color, and show the caregiver’s full face.

Note: Your Medical Marijuana Registry ID card expires after one year and must be renewed 45 days before expiration.

Eligibility Criteria

According to the Office of Medical Marijuana Use, medical marijuana caregivers in Florida must:

  • Be 21 years of age.
  • Be a Florida resident.
  • Agree in writing to assist with the qualified patient’s medical use of marijuana.
  • Be registered in the Medical Marijuana Use Registry of no more than one qualified patient.
  • Complete a caregiver certification course provided by the Florida Department of Health or its designee.

If a caregiver is not a close relative of the qualified patient they are assisting, they must also pass a background screening.

Reciprocity

Florida does not offer medical marijuana reciprocity to visitors with a medical card from other states.

How much does a Florida medical marijuana certificate and card cost?

Please note: We can only offer online consultations to existing Leafwell patients for renewals. First-time patients or those new to Leafwell can join our waitlist to be notified when an in-person medical marijuana certification pop-up is happening near you. In-person certifications will cost $149.

An online renewal consultation for existing Leafwell patients in Florida costs $99 and includes a 30-week certification. You’ll only be charged if approved.

Patients will also pay a separate $75 application fee (+ $2.75 processing fee) to the Florida Office of Medical Marijuana Use.

How old do I have to be to apply for a medical marijuana card in Florida?

You must be at least 18 years old to apply for a Florida medical marijuana card.

Minor patients may apply with consent from a custodial parent or legal guardian.

Caregivers must be 21 years old.

Can I be/have a caregiver for a medical marijuana patient in Florida?

Yes, medical marijuana patients in Florida can have a caregiver. Caregivers will complete their own application form and must also be listed on the patient’s application form. Caregivers and patients must renew their physician certification 45 days prior to the card expiration date.

Caregiver Requirements:

  • Be a Florida resident and 21 years of age or older.
  • Not be a qualified physician.
  • Not be employed by or have economic interest in Medical Marijuana Treatment Centers (MMTCs) or a Certified Marijuana Testing Laboratory (CMTL).
  • Successfully complete the caregiver certification course within the Medical Marijuana Use Registry every 2 years.
  • Pass a background screening, unless the medical marijuana patient is a close relative of the caregiver.
    • After June 12, 2023, all NON-CLOSE RELATIVES applying for a caregiver Medical Marijuana Use Registry identification card must complete a level 2 background screening. More information can be found here.
  • Be registered to no more than one qualified patient, unless:
    • The caregiver is a parent or legal guardian of more than one minor who is a qualified patient.
    • The caregiver is a parent or legal guardian of more than one adult who is a qualified patient and who has an intellectual or developmental disability that prevents the patient from being able to protect or care for himself or herself without assistance or supervision.
    • All qualified patients the caregiver has agreed to assist are admitted to a hospice program and have requested the assistance of that caregiver with the medical use of marijuana; the caregiver is an employee of the hospice; and the caregiver provides personal care or other services directly to clients of the hospice in the scope of that employment.
    • All qualified patients the caregiver has agreed to assist are participating in a research program in a teaching nursing home pursuant to s. 1004.4351, Florida Statutes.

Here’s how to apply as a caregiver:

  • Retrieve your login credentials for the Medical Marijuana Use Registry from your email.
  • Access the “Your Card” page in the Medical Marijuana Use Registry to complete your online application.
  • Supply all necessary documentation and submit your payment information for the $75 processing fee, plus a $2.75 convenience fee.
  • Once your application is approved, you will be sent a Medical Marijuana Registry ID card. You can check the status of your application while it is under review.

How do I apply for the state program in Florida?

Have the following ready before you begin your application:

  • Photo ID The Registry is connected to the Florida Department of Highway Safety and Motor Vehicles’ (FLHSMV) demographic database. If there is a match to the information in your profile, your Florida ID card or driver’s license photo can be automatically added to your application and approved.
  • Proof of Residency Documentation
  • $75 payment fee (Plus $2.75 convenience fee)

How do I apply for the state program in Florida?

  • After your appointment with a Leafwell physician, you will receive 2x emails with the subject line “Medical Marijuana Use Registry New User Account” (here’s a walkthrough for step 1).
    • One email contains your username (your email address).
    • The second email contains a temporary password – these passwords are case-sensitive.
    • Click the link in the email to log into the Medical Marijuana Use Registry https://mmuregistry.flhealth.gov/.
    • Click login in the top left and then enter your email, and password and click the login button.
    • Type the temporary password into the ‘current password’ box, then set your own.
    • Once this is done, you will be automatically redirected to your registry homepage.
  • Click on “Your Card” and then click “Begin Initial Application.”
  • Click “Supply a Photo” to supply either your State ID photo or a passport-style photo to your application.
    • Clicking “Check My State ID” will upload your driver’s license photo from the Florida Department of Highway Safety and Motor Vehicles (FLHSMV) demographic database.
    • Clicking “Supply My Own” will allow you to upload your own photo from your device. (Using the FLHSMV sourced data will automatically approve the photo and may cut down on processing time, while supplying your own photo may increase the processing time.)
  • Scroll down to the “Proof of Residence” section and click “Check State Residency Status” to obtain your Driver’s License or Florida State ID from FLHSMV, or click “Supply My Own” to supply your own proof of residency document.
  • Review all data to make sure it is accurate. Then click “Apply.”
  • Electronically sign your application and click “Submit My Card Application.”
  • Pay the application fee.
  • Click “Print for your Records” to print the payment confirmation for your records.
  • The entire application review process takes a total of 10 business days from the date of submission. Once the application is approved, you will receive an electronic temporary card to use while the physical official card is printed. The official card will be mailed to you within five business days.
